Voting Representative Form
This form must be completed by each AD and signed off by all head coaches on or before October 1st of each new school year and sent to the secretary of the Conference.

Realignment Calculation Form
This form must be completed and submitted to Tom Pagano to gather information on divisional realignment.
FOOTBALL CROSSOVER GAME WITHDRAWL FORM
Crossover games, which do not affect divisional standings, may be dropped by mutual agreement of member schools prior to the start of the practice season for FOOTBALL. The cancellation agreement must be submitted in writing on the form provided, by both schools, to the President of the Shore Conference/GMC and approved by the same.

Athletic Department Information Form
This form is used at the start of each year to update the secretary of any changes in information concerning your specific athletic departments information.
Championship Plaque Request Form
Request form to be completed when one of your schools teams wins a Shore Conference Divisional Championship.
Shore Conference Tournament Withdrawl Form
Once qualifying for a specific Shore Conference Tournament you must submit this form if you are removing your team from participating. This must be submitted five (5) days prior to the seeding meeting.
